Rinse the pearl barley under cold water to remove any dirt and impurities.
In a large pot, combine the rinsed barley with 2 liters of water.
Bring to a boil over high heat, then reduce the heat to low and let it simmer uncovered for about 45 minutes, or until the barley is tender and the water has reduced.
Once cooked, strain the barley water into a large bowl using a fine sieve; you can save the cooked barley for another use.
Let the barley water cool to room temperature.
Squeeze the juice of one lemon into the cooled barley water and stir well.
Add 2 tablespoons of honey to the lemon barley water and mix until fully dissolved. Adjust sweetness to taste.
For additional flavor, add 5 fresh mint leaves and let them steep for several minutes.
